Neill revealed in March 2023 that he had been diagnosed with angioimmunoblastic T-cell lymphoma. In an interview with Australian Story in October 2023, he said, "I’m not afraid of dying. It never worried me at all." However, he expressed his hopes for life by stating, "I still have things I want to do."
Starting his career at a young age, Neill gained attention with Sleeping Dogs, one of the first New Zealand films to be released internationally in 1977. He also starred alongside Judy Davis in My Brilliant Career in 1979 and brought the character Damien to life in The Final Conflict in 1981.
